What Is This Settlement About?

A proposed class action settlement has been reached with Lakeview Health Systems, L.L.C. in Skov et al. v. Lakeview Health Systems, L.L.C., Case No. 16-2024-CA-003154-AXXX-MA, pending in the Circuit Court of Duval County, Florida. The case concerns a targeted cyberattack on Lakeview Health's computer systems discovered on or around January 25, 2024 (the "Data Incident"), in which certain files containing private information were accessed. Those files may have contained names, addresses, dates of birth, Social Security numbers, driver's license numbers, financial account numbers, patient IDs, medical treatment information, diagnoses, conditions, prescription information, and health insurance information.

Lakeview Health denies any wrongdoing and the court has not decided who is right. Both sides agreed to settle to avoid the cost and uncertainty of continued litigation. If you were notified that your information may have been involved in the incident, you may be eligible to file a claim for cash benefits and credit monitoring before the August 24, 2026 deadline.

Who Qualifies?

You may qualify if you received notice that your personal information or protected health information may have been involved in the January 2024 Lakeview Health Systems data security incident. The settlement class covers the roughly 10,772 individuals identified by Lakeview Health as potentially affected.

How Much Can You Get?

Eligible class members can choose between documenting their losses or taking a flat cash payment:

• Ordinary expenses: reimbursement of up to $2,000 for documented out-of-pocket costs fairly traceable to the data breach (for example, bank fees, postage, or the cost of credit reports and credit freezes).
• Extraordinary losses: reimbursement of up to $5,000 for documented losses from actual identity theft or fraud connected to the breach.
• Lost time: up to 4 hours of time spent dealing with the breach, paid at $20 per hour (up to $80).
• Credit monitoring: class members may enroll in one year of CyEx Medical Shield Total at no cost, which includes $1 million of medical identity theft insurance and monitoring for healthcare insurance ID, Medical Record Number, and Health Savings Account activity.

Combined, the documented losses and lost time can total up to $7,080, and that path also includes the credit monitoring benefit. If you do not want to gather documentation, you can instead elect a flat $50 alternative cash payment with no proof of loss required — but the $50 alternative is taken in place of credit monitoring and the other cash benefits, not in addition to them. You may choose only one cash path. Payment amounts may be adjusted pro rata depending on the number of valid claims filed.

How to File a Claim

The fastest way to file is online through the official settlement website. You can also request and mail a paper claim form. To file, you will generally need information from your notice (such as a Notice ID or Claim ID, if one was provided) along with any documentation supporting an ordinary or extraordinary loss claim.

Important Deadline

The deadline to submit a claim is August 24, 2026 (online submissions and mailed claim forms must be postmarked by that date). Class members who want to be excluded from, or object to, the settlement must do so by July 23, 2026. The Court's final approval hearing is scheduled for October 8, 2026 at 10:00 a.m. Eastern Time via Zoom; that date may change, so check the official settlement website for updates. Settlement payments are distributed only if the Court grants final approval and after any appeals are resolved.
